About Company

Community Health Partnership of Illinois (CHP) is a Federally Qualified Health Center (FQHC) whose mission is to build and deliver quality, culturally responsive healthcare to our patients, caregivers and the communities we serve. CHP empowers individuals, including the uninsured, underserved, migrants and seasonal agricultural workers, to attain their best health and well-being. Our network is comprised of six health and dental clinics and multiple outreach sites across northern and central Illinois.

Our services are designed to create access to quality primary care services for the uninsured and underserved. The vast majority, 99 percent of CHP patients, live in low-income households (less than 200 percent of the Federal Poverty Level (FPL)) with over two-thirds (67.2%) of families that live with poverty-level incomes (100% FPL). Three-quarters of our CHP patients do not have health insurance, which is in alignment with our mission that no one is denied services based on their inability to pay.

Our patients experience high rates of chronic disease, including hypertension, diabetes and asthma. Our approach to care is based on tools of empowerment to meet the primary care needs of our patient population and to reduce health disparities. Our award-winning Promotores de Salud and bilingual-bicultural Outreach Teams conduct robust health outreach, screening and service navigation for care that targets agricultural workers, their families, and other uninsured and underserved community residents.

A full complement of culturally and linguistically responsive medical, dental and behavioral health services are offered at our fixed and mobile services sites in Aurora, Harvard, Kankakee, Mendota, Princeville and Champaign, Illinois, by a very diverse staff. We are committed to providing healthcare with dignity, kindness and fairness to address the healthcare needs of medically underserved in our rural communities.
